The first thing I would do is connect it to iTunes and do a restore. When its finished it will ask you if you want to set up as a new phone or if you want to restore from a backup. You will want to set it up as a new phone. Do not use a backup. Also do not install any third party applications on it. Contact Apple Support if it continues, and you have a warranty.
